Sport Registration

How to Register for a Sport

 

In order to be eligible to play a NHIAA sanctioned sport, every athlete must complete the following 4 steps. 

  1. Family ID Registration (electronic filing of insurance, health information and emergency contact info, etc.)  CLICK HERE for the Family ID Website.
  2. Physical Form (good for 2 years, cannot expire during the season of play, and must be kept on file with Athletic Dept.) CLICK HERE for the ConVal Physical Form (your Doctor's Universal Physical Form is also accepted). Athletics Fax: 1-603-925-4151.
  3. ImPact Test (administered every 2 years at the HS and kept on file by Athletic Dept.) Take the online Impact Test by clicking on this link [CVHS Code: e30340Cf98 - click Validate, click Launch Baseline Test].
  4. Participation Fees for each sport. CLICK HERE for more information on participation fees, individual and family caps. 

Where are the ImPact Tests administered?

The athlete can take the online Impact Test by clicking on this link [CVHS Code: e30340Cf98 - click Validate, click Launch Baseline Test] if you have any questions or need to take the impact test in person please email Chris, Athletic Trainer (convalatc@gmail.com).

Where does my athlete confirm Eligibility?

Athletic eligibility is confirmed in the Athletic Office or by your coach.

Is Insurance required?

Student-Athletes must have medical insurance to participate in interscholastic athletics at CVHS (School accident insurance is available - please call school for information).

When are try-outs?

Sport Try-outs begin on the first practice date.

Other Important Information about Tryouts:

Students must attend try-outs to be included on a sports team.

Practice and game schedules will be given to athletes by coaches at start of their respective seasons.
